Appendix B, the Preceptor Application & Agreement, is the online form your preceptor completes so Graceland University can approve them for one specific practicum. The preceptor fills it in, not you, and a new one is needed every term they work with you. It also sets up the Typhon account your preceptor uses to evaluate you.

Who completes Appendix B, and how often

Your preceptor completes Appendix B. Graceland describes it as the application and agreement used to approve the preceptor for each practicum, and it wants a new one for every term or practicum in which that preceptor works with you. A nurse practitioner who supervised your NURS6740 in Fall and returns for part of NURS6830 later fills it in a second time.

It travels in step with your Appendix A. You file the site request; your preceptor files the application; a contract coordinator pairs the two, checks the site contract and your Castle Branch record, and only then treats your file as ready. Both forms fall under the same paperwork deadline for your term.

The form is a track-specific online form on my.graceland.edu, and the handbook prints a separate link for FNP, AGACNP and PMHNP. Unlike Appendix A, it opens without a Graceland account, which makes it easy for a busy clinician to finish between patients.

The details your preceptor needs from you

The application asks for facts about you that only you can supply. Put them in one short message so your preceptor can complete the form in a single sitting.

Attach Graceland's one-page Preceptor Information and Responsibilities sheet, which the handbook includes for students to hand to a prospective preceptor. It lists what Graceland expects, from orientation and feedback to reviewing your differential diagnoses and completing evaluations when you send the request, so your preceptor agrees with the full picture in view.

What Graceland weighs on the application

Appendix B is how the lead faculty and contract coordinators decide whether this clinician fits this course. The handbook's review points include credentials and a verified license, board certification or eligibility for physicians, experience in the APRN role, the number of patients seen in a day and whether that patient mix matches the course. Graceland expects students to see one to two patients an hour, so a light schedule can count against a preceptor.

The same handbook gives two experience figures. Its review criteria on p. 58 mention 1-3 years in the APRN role, while the qualifications list on p. 60 asks MSN-prepared NPs for at least one year. If your preceptor sits near that line, ask your track coordinator before the form goes in.

Physician assistants may not precept any Graceland practicum hours, and PMHNP students work only with psychiatrists or nurse practitioners certified as PMHNPs. The preceptor requirements page covers each track in full.

The Typhon account that comes with it

Appendix B asks your preceptor for an email address unique to them. Graceland uses it to create the preceptor's Typhon account, and the handbook tells preceptors to expect one or two evaluations in Typhon during the term.

Those evaluations carry weight for you. Every preceptor files a midterm evaluation and a final one and signs off your time logs as the term goes, and Graceland can stop you from progressing in a course when evaluations are late. A personal work address, rather than a front-desk inbox, keeps those requests in front of the right person.

After approval, your name, your preceptor and your site appear in Typhon, entered by a contract coordinator no later than week one of the term. The Typhon page covers case logs and keeping your records.

Paper copies, substitutes and a second preceptor

Graceland built Appendix B as an online form. When a preceptor returns a completed copy by email or on paper instead, it belongs in the contract coordinators' inbox, soncontracts@graceland.edu, where Graceland routes every practicum message.

Short cover needs no application. Graceland lets one substitute a term step in at your primary site for a maximum of 40 hours with no Appendix B. Past 40 hours, the extra time counts only once that clinician has an approved application, which makes them your second preceptor.

Adding a second preceptor means a second Appendix B, and Graceland turns away requests for an added second preceptor during the final two weeks before a term opens. Our page on changing or adding a preceptor covers what follows.

One Appendix B covers a preceptor across several offices in a term, but every office still needs its own Appendix A and its own site contract.

When NPHub makes the placement

Students who opt in to NPHub, Graceland's optional paid partner, do not collect Appendix B themselves. According to the handbook, NPHub gathers the affiliation agreement, Appendix A and Appendix B from the site and preceptor and gives them to Graceland, which checks again that the preceptor matches your course and that the paperwork is complete.

Graceland's approval on that route covers the preceptor and site. Final clearance still waits on your Castle Branch documents, the drug screen and the background check, the same as for any student. Does my preceptor need a Graceland account to complete Appendix B?

No. The handbook tells preceptors they should not need to sign in, and the form opens from a track-specific my.graceland.edu link. Send your preceptor the link for your track together with your name, student ID, Sting email address, course name and term.

My preceptor completed Appendix B last term. Do they need to do it again?

Yes. Graceland asks for a new Preceptor Application & Agreement for each term or practicum in which a preceptor is used. Keep in mind that one preceptor can be used for at most two of your three practicums, so a returning preceptor often pairs with a new one for the third.

Can my preceptor email or print the form instead of using the online version?

The online form is Graceland's standard route. Completed copies sent by email or on paper go to soncontracts@graceland.edu, the contract coordinators' mailbox. Confirm with the coordinators that the copy arrived, because your term's paperwork deadline covers Appendix B as well as your own forms.

What does a preceptor do in Typhon for a Graceland student?

The preceptor uses the account created from Appendix B to complete a midterm and a final evaluation, and Graceland asks preceptors to validate your time logs along the way. You send each evaluation request; the preceptor completes it in Typhon. Late evaluations can block your progress in the course.

Does a substitute preceptor need Appendix B?

Not for brief cover. One substitute per term may supervise you at your primary site for up to 40 hours without it. Any hours beyond 40 count only after the substitute completes an approved application and becomes your second preceptor.
